Is Your Dental Practice Growing or Just Getting Busier?

Four real dental practices. Four different growth stories. What the numbers reveal about 2027.
A packed schedule feels good. The phones are ringing, the team is busy, and the days fly by. But is all that activity translating into higher production, more active patients, and a stronger year in 2027? As 2026 winds down, it’s worth looking beyond how busy your practice feels and examining what the numbers say about your growth.
The latest Q3 findings from the ADA Health Policy Institute paint an interesting picture. Some 38% of dentists report they’re not busy enough (and only 8% say they’re too busy to treat all patients requesting care), while confidence in their own practices has dropped from 68% a year ago to 50% today. The report also indicated that dental care spending continues to lag behind overall healthcare spending. Not exactly the momentum dentists are looking for heading into 2027.
There are hidden costs of standing still, and that’s what creates the great dental growth divide between flatlining locations and those that are pushing ahead.
The following four charts are from real clients who work with us and are connected to Practice ZEBRA. These are offices that run a variety of marketing campaigns, including SEO, Google Ads, and direct mail. These graphs demonstrate the positive impact of investing in the future and what that can look like. It’s not about perfection. Growth rarely happens in a straight line. Seasonal fluctuations, staffing, scheduling, patient demand, and market activity all influence results. What matters is the longer-term trajectory. New patients are coming. Is production following?
It’s never a perfectly smooth upward trajectory, but when everything starts working together, targeted marketing hits, teams are trained in call handling, and the office is available to book patients in quickly, then the right things happen. This has led to steady improvements in new-patient flow for this location, putting them on track for a 12% year-over-year increase in production.

Key insight: marketing generates opportunities. Scheduling availability and effective call handling help turn those opportunities into production.
Already matched last year’s production. In September.
The next graph is from another office. When there is a steady flow of new patients, active patient counts trend upward. This practice worked hard on nailing down just the right feel for their website, and they work hard to generate a very consistent flow of positive patient reviews, instilling confidence in new prospects looking at their practice. The impact is paying off with solid practice growth. As of the end of September, they had achieved $1.5 million in production, the same number they hit for 2025, putting them on trajectory for a 33% year-over-year revenue increase. Nice.

Key insight: Building an active patient base supports sustained production growth, enabling reinvestment in practice growth, CE, and new technologies to continue attracting higher-quality patients.
Winning the battle against patient attrition.
The following practice launched a more dedicated marketing effort in April of 2026, including a website upgrade and targeted direct mail, and they’ve seen a net gain in patients, helping to outstrip attrition. While they are on track to have a modest production increase over the previous year, the key for this location is to remain consistent in their efforts to add more new patients and increase production by improving call handling and case acceptance.

Key insight: New patient acquisition must exceed patient attrition to grow the active patient base.
The rock stars who don’t stop. The long game. $11 million in production.
Now, let’s look at a super-solo practice that’s been investing in marketing with us since 2018. The cumulative result has generated over 48,000 leads, over 4,000 new patients, and $11 million in production, resulting in a 35X return on marketing spend.

Key insight: Sustained marketing investment can generate substantial cumulative results, even when monthly performance fluctuates.
What will your numbers say about 2027?
Four practices. Four different growth stories. And one thing in common: they’re using data to understand what’s working, identify opportunities, and make smarter decisions about their future.
The question isn’t whether your practice had a perfect year. It’s whether you’re building momentum to have a stronger year ahead.
